Not everything may be explained there, but please take a look at

Section 9. Semantics of mathematical symbols
Section 7. Semantic editing facilities

Most of the subtleties are not supported by TeX/LaTeX;
that is one of the reasons why TeX/LaTeX is inappropriate in my opinion.
