I looked at the generated files in /tmp. If I generate the dvi/eps/pdf files `by hand', I get the same results. For some reason, dvips gives the following error:
dvips: Can't make it EPSF, sorry
I can't figure out why. There's really only one page in equation.dvi.

Figured it out. I had an old ~/.dvipsrc that specified letter size paper. Deleting this file solved the problem. Apologies for the false alarm. Equation Editor works great.

Thus it seems this problem is related to an obscure option
remaing in your .dvipsrc (or in some similar location).
