Your inclusions should use a Part or Chapter tag, not a Title tag.
For the numbering issue I once saw a problem in a TOC which was caused by
having nested different section tags (like <section|<paragraph| xxxx yyyy
zzz>>), but it's hard to be sure without seeing an actual file.
