Note that the icon resolution is not optimal: a qt plugin has been 
forgotten in this package.

Did you restart TeXmacs to get the changes?

What are the values of the preferences "use native menubar" and "new 
toolbar" in your ~/.TeXmacs/system/preferences.scm file?

It seems to work for me. Could you please check if your shortcuts 
exactly match the scheme commands of the menus?

For instance, if your shortcut for "Small figure" is (make 
'small-figure), then it won't appear since the menu command is

(wrap-selection-small
   (make 'small-figure))

The code was disabled because no icon was provided, and it made the tab 
widget quite large. I've tried to remedy these issues.

I think I've fixed this issue in svn version #15104.
