I remember having had a similar problem about one year ago in
a complex document. However, after some debugging, I found out that
the error was in the document itself. I don't remember exactly,
but I think that I used some labels/references/layout markup inside
the section titles and that I had some strange index too.
If you cannot find the problem yourself, then you may send us
your document and we can take a look at it.
