The problem is: you inserted the figure label inside the figure 
caption. Thus every label appears *twice* in your document: once inside 
the caption, once inside the caption in the figure list. You should 
move the label away from the caption, but still inside the figure 
"float".

The same problem appears when you put the label inside a section title 
and then insert a summary.
